Our way of working with people is to meet their immediate care needs while also supporting their goals.Through our commitment to person-centered care, we work closely with each patient and their family to create a care plan that allows for the highest level of independence possible. Whenever possible, we honor each person’s preferences for how they want to live.

We believe that personal growth, self-responsibility, and independence are key parts of wellness and recovery—especially after life-changing events.

The people we serve are involved in choosing their care team, and we make every effort to keep that team consistent. A nursing supervisor meets with each new client and their family to create a care plan based on the doctor’s recommendations. Regular meetings are held with the nursing supervisor, the client, their family, and caregivers, along with hands-on evaluations, to ensure care is truly a team effort.
